Medical Forms and Needs.  Each scout and adult going to summer camp will need to complete BSA medical forms part A, B and C.  Part C must be signed by a doctor or other medical professional included in the BSA description.  That description and the medical forms can be found at:http://www.scouting.org/scoutsource/healthandsafety/ahmr.aspx.  We would like to have all medical forms completed and turned into Greg Leblanc by May 15, 2013.  When you turn in the medical forms, please include a description of any allergies your scout has and any medications you scout will need to take during summer camp.  Please let Greg or Steve Hunting know if you have any questions.

3/21 - To All,

One addendum I would like to make about the BSA Medical - I also need a copy of your insurance card with the medical form. When you copy the insurance card, please copy the front and back of the card onto one side of one page of paper.Please do not copy the front of the card onto 1 page and the back onto a second page and then staple it together.

Just as a reminder, are targeting a turn in date of May 15, 2013 for the medical forms and insurance cards. I am sure there will be some exceptions due to when you can get in for the physicals based upon insurance - do the best you can.

If your son takes prescription meds, please let me know and I will get you a blister pack so you can fill it and bring during the final check in time before we leave.
